Generate is opt-in (HARD RULE): Do not load references/phases/generate/generate.md unless the user chose option C at the post-Estimate Decision gate (estimate.md), accepted the decide-complete resume offer, or the user’s current-turn message is an explicit request to produce Terraform / migration scripts (not merely mentioning Terraform).
